Grading Of Tonsils

Tonsils are two oval, lymph nodes in the back of your mouth and the top of your throat. They serve the purpose to fight germs that enter the body through the mouth or nose. The grading of tonsils is based on their size and degree of inflammation. The most commonly used grading system is the Brodnitz classification.

Brodnitz Classification

  • Grade 0: Tonsils are completely embedded in the tonsillar fossa and are not visible.
  • Grade 1: Tonsils are visible, but less than 25% of the tonsillar fossa is occupied.
  • Grade 2: Tonsils occupy 25 – 50% of the tonsillar fossa.
  • Grade 3: Tonsils occupy 50 – 75% of the tonsillar fossa.
  • Grade 4: Tonsils are large enough to occupy more than 75% of the tonsillar fossa or touch the uvula.

Modified Brodnitz Classification

This classification system also takes into account the degree of inflammation:

  • Grade 1 – 4: Same as the Brodnitz classification.
  • +: Indicates mild inflammation.
  • ++: Indicates moderate inflammation.
  • +++: Indicates severe inflammation.

Accurate grading of tonsils is essential for:

    To determine the severity of tonsillitis.

    To decide on the best course of treatment, such as antibiotics or surgery.
